Synopsis "Indice Pronostique Proposé Pour La Mortalité Postopératoire (in French)"

Introduction. L'hématome intracérébral spontané est la forme d'accident vasculaire cérébral qui présente le taux de mortalité le plus élevé parmi les maladies cérébrovasculaires dans le monde. Il n'existe pas de consensus sur la sélection des patients devant subir un traitement chirurgical afin de réduire la mortalité postopératoire. Objectif. Proposer un indice pronostique de mortalité postopératoire pour les patients présentant un hématome intracérébral primaire traité par craniotomie et évacuation de l'hématome. Méthodologie. Une étude explicative, analytique et prospective a été réalisée sur 127 patients présentant un hématome intracérébral. Les facteurs préopératoires liés à la mortalité postopératoire ont été identifiés et un indice pronostique a été créé. Nous avons déterminé si l'index pronostique prédisait correctement la mortalité postopératoire. Résultats. Les facteurs suivants ont été identifiés comme prédicteurs de mortalité âge >= 60 ans, EGC = 1,25 centimètres. L'index pronostique créé a permis de classer les patients en risque de décès faible, intermédiaire, élevé et très élevé. Conclusions . Un indice pronostique est proposé qui a un bon pouvoir discriminant entre les patients vivants et décédés.
