Akademische Arbeit aus dem Jahr 2020 im Fachbereich Gesundheit - Gesundheitswesen, Sprache: Deutsch, Abstract: Im deutschen Gesundheitssystem sind im Gegensatz zu vielen anderen Ländern die ambulante und stationäre Gesundheitsversorgung weitgehend getrennt. Seit Jahren wird diskutiert, ob durch eine Aufhebung dieser starren Sektorengrenzen eine bessere oder zumindest gleichwertige Patientenversorgung bei gleichzeitiger Kostenreduktion erreicht werden könnte. Mittlerweile gibt es zahlreiche Ansätze für eine sektorenübergreifende Gesundheitsversorgung. In dieser Arbeit werden die ambulante und stationäre Versorgungsstruktur kurz vorgestellt, einzelne bereits bestehende sektorenübergreifende Versorgungsmodelle, den ambulanten und den akut-stationären Versorgungsbereich betreffend dargestellt und versucht, soweit möglich, die Qualität und Effektivität dieser Modelle zu beurteilen. Im Anschluss folgt die Vorstellung neuer sektorenübergreifender Versorgungsansätze. Abschlie end werden Chancen und Problematiken der sektorenübergreifenden Versorgung diskutiert. Abstract In contrast to many other countries, outpatient and inpatient health care are mostly separated in the German health care system. For many years it has been discussed whether a remove of these rigid sector boundaries could lead to better or at least equivalent patient care with a simultaneous reduction in costs. There are now numerous approaches to cross-sector healthcare. In this work, the outpatient and inpatient care structure is briefly presented, individual existing cross-sectoral care models are presented, regarding the outpatient and acute inpatient care area, and attempts are made to assess the quality and effectiveness of these models as far as possible. This is followed by the presentation of new cross-sectoral care approaches. Finally, opportunities and problems of cross-sector care are discussed.
