Sobre la base de la teoría del aparato psíquico desplegada en El yo y el ello, Freud reelabora conceptos presentes desde el origen, como la defensa y la represión. Se otorga un lugar importante a la neurosis obsesiva y se reexaminan dos historias de fobia: la del pequeño Hans y la del Hombre de los Lobos. Sale a la luz una nueva configuración, según la cual es la angustia la que provoca la represión y no a la inversa. En última instancia, el prototipo de toda situación ulterior de peligro lo constituye el trauma del nacimiento; la angustia reaparecerá cada vez que haya un peligro considerable, bajo la forma de angustia de la pérdida de objeto.
